On average, a patient’s first visit lasts about an hour.
Please arrive 15 minutes early to sign in, complete your paperwork, and/or change into comfortable clothing, if necessary. Filling out paperwork before your initial evaluation appointment will speed your check-in time. You can find our new patient forms above.
On your first visit, you’ll need to bring your physician referral or prescription (if required by your insurance), your insurance card, your new patient registration forms, your ID or driver’s license, and your co-payment (as applicable). If desired, you may also bring comfortable clothing to change into for the session.
We want you to leave your first appointment with us feeling better than when you walked in.
At your first visit, your physical therapist will conduct an initial evaluation and discuss your plan of care. Information obtained during your initial visit with us is used to set goals for your continued treatment. Your physical therapy goals may include:
- Improved movement, strength, endurance, and/or flexibility
- Decreased pain
The focus of your subsequent visits will be treatment (based on your diagnosis and treatment goals).
Immediately following your initial evaluation, we will schedule you for your follow-up appointments. We are always excited to get started and see you reach your goals and get you back to living the life you want. You can also schedule your own appointment HERE! We look forward to helping you advance your health!
If you are insured, and your insurance requires a co-pay or deductible, that will be due at the time of your visit with us.
